INFORMATION COLLECTED

Information may include your name, mobile number, and message content.

USE OF INFORMATION

Information is used solely for care coordination, scheduling, and administrative

communication. SMS communication is limited to brief, non-urgent interactions.

INFORMATION SHARING

No mobile information will be shared with third parties or affiliates for marketing or

promotional purposes. Information may be shared with HIPAA-compliant service

providers as necessary for operations or as required by law.

SECURITY

SMS messaging is not encrypted and may be intercepted or accessed by others. By

opting in, you acknowledge and accept these risks.

DATA RETENTION

Text messages may be retained as part of your clinical or administrative record in

accordance with legal and professional requirements.
